We do not collect a Social Security number through this website, and we do not accept identification, immigration or tax documents through this website — bring them to your appointment or ask your agent for a secure link to send them.

We use this information to contact you about your request and to help you with Marketplace, dental or vision enrollment. We do not use it for any other purpose.
